Well-led
The management team and staff had worked with an external agency of consultants, and the local authority, to make the improvements required at the service . The service also worked well alongside a variety of external health professionals, to ensure good outcomes for people. The management and staff team used consistent local services for people, and this consistency also provided continuity of care. This included the same team of district nurses visiting, and regular and flexible support from the local GP service.

Partnerships and communities
The service worked well with a range of professionals to ensure people received safe and effective care. There were effective processes in place to make sure staff worked with healthcare partners to support people’s wellbeing.
