Effective
We looked at whether people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. We found that people’s outcomes were consistently good, and people’s feedback confirmed this. People's needs were assessed and reviewed regularly. Staff supported people to live healthy lives. Care and treatment was evidence based.

Assessing needs
People's needs and risks were assessed and reviewed regularly. Assessments took account of pertinent risks and were person centred.
Staff had good knowledge about people's needs and people and relatives told us they thought staff knowledge was good.
The registered manager and other senior leaders completed pre-admission assessments followed by more thorough assessments. These were reviewed regularly. Outcomes such as falls and weight loss were monitored and recorded, with actions taken when necessary.
Delivering evidence-based care and treatment
We spoke to staff and found that staff had good knowledge about people's wider health needs and how to support them.
Care records we reviewed reflected good practice in relation to people’s care needs, risk management and preferences. Staff felt confident using the electronic care record system and could use hand held devices to input notes timely. Staff were knowledgeable about people’s needs and were able to describe how they supported them.
The provider used an electronic care planning and recording system. This included up to date information from other professionals, including speech and language therapists, dieticians, falls prevention team and social workers. Senior staff had oversight of records and could identify when further action needed to be taken. Information about people’s nutritional needs had been provided on each floor to guide staff about people’s support needs. Staff followed nutritional guidance and recorded the quantity of food and drink people assessed as at risk, consumed. Care records were kept up to date by staff and were reviewed and updated by senior staff.

Supporting people to live healthier lives
People told us they were supported with their wider health needs. One person said, "The home look after my medications and GP appointments." Another person said, “If I am not well the girls will get the doctor if necessary. I don't have to worry." Relatives also spoke positively; one relative said, "I work very closely with the staff. We always discuss my wife's nutritional needs. I do trust the home with medication and appointments for my wife." People were supported by activities coordinators who encouraged people to participate in activities throughout the day, both in the lounge and people's own bedrooms.

The provider had close links with local health teams. The provider's processes meant that if someone's health deteriorated, staff knew who to contact and what action to take, for example speech and language teams.
