Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last inspection we rated this key question requires improvement. At this inspection the rating has improved to good. This meant people were safe and protected from avoidable harm

Learning culture

Score: 3

The service had a proactive and positive culture of safety, based on openness and honesty. They listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. We saw examples of incidents being thoroughly investigated. If needed, corrective action was taken, such as additional staff training being undertaken.

Safe systems, pathways and transitions

The service worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. People were supported in the community, however staff jointly worked with people’s occupational therapists and district nurses to ensure referrals were made when needed.

Safeguarding

The service worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. They concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The service shared concerns quickly and appropriately. For example, staff told us the action they would take if they thought someone was being harmed or abused. People told us they felt safe. One person said, “I feel safe in their hands." Also, “They tell me who is coming, so I always know who to expect.” A relative told us, “I really feel they care about [family member] they are really attentive to her needs, and they support her and contact us if there is anything wrong.”

Involving people to manage risks

The service worked with people to understand and manage risks by thinking holistically. They provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. For example, people or the family they lived with were involved in their risk assessments. We saw how 1 person was concerned about slipping on a wet bathroom floor. They asked staff to dry the floor after bathing, to reduce the risk of falls. We saw this was recorded in the care plan.

Safe environments

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Each person’s home had been risk assessed with their permission, for any potential hazards. This helped the person and staff use the safest environment possible.

Safe and effective staffing

Score: 3

The service made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. For example, staff said they felt their rotas were always completed fairly, and they never had to rush around. One person told us staff could be late occasionally. However, everyone else said staff came on time. Rotas allowed for travel time and calls were evenly spaced. One person said, “They can’t always help traffic, but they always let me know if they are running late."

Infection prevention and control

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Staff had access to personal protective equipment (PPE)which they wore when providing personal care.

Medicines optimisation

Score: 3

The service now made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. They involved people in planning, including when changes happened. Medicines were now well documented and audited. Staff had now undertaken extensive medication training.
