Effective
We assessed delivering evidence-based care and treatment within this key question. Our rating for this question remains good. Relatives told us they were involved in assessing people’s needs and were able to state their loved ones choices and preferences as to how their care and support was provided. People’s care and support was planned and delivered in line with guidance provided by professionals and best practice.

Delivering evidence-based care and treatment
Relatives told us they felt involved in their loved ones care. One relative told us, “We have been thrilled with their progress and level of care. They are involved in their daily life and choices.” Another relative told us, “They get some good meals including their favourite food.” We observed staff giving choice and promoting independence to people, while providing support that was person centred.
Staff told us they knew where to find information about people’s nutrition and hydration within electronic care plans. Staff told us people’s care plans contained relevant and up-to-date information.
Records showed a person-centred approach that highlighted people’s likes and dislikes and what was important to them. Care records contained sufficient information to provide guidance to staff to support people’s specific needs around food and drink. Care plans included information from professionals, such as dieticians. Best practise national guidance on supporting people with swallowing difficulties was available to staff and accessible.
