There was a strong, positive culture within the leadership of the service which was devoted to learning and innovation. In this key question we looked at the quality statements shared direction and culture, capable compassionate and inclusive leaders, and learning improvement and innovation.

Shared direction and culture

There were processes to promote a listening culture throughout the service. For example, we saw a staff survey was completed and an action plan was put in place to address any areas for improvement. For example, we saw one area for improvement was around staff support and appreciation. We saw actions had been taken in response to this such as the manager purchasing thank you gift packs and giving staff personal thank you cards. The service had a strong equality, diversity and inclusion policy in place, which states: ‘We see equality, diversity, and inclusion not only as part of our inclusive culture, but essential to our continued success.’ We saw this was put into practise in every aspect of people's care, for example in the way people were treated as individuals, how different religions were celebrated such as the Diwali festival, and how adaptations were made to ensure people were able to access certain activities.

The registered manager, deputy manager and staff described how people were at the centre of the service and they provided many examples throughout the assessment. People were running the home with the support of the staff and management. The manager talked us through all the culture work he had completed. Observations and feedback from people, relatives and a health professional supported what we were being told.

Capable, compassionate and inclusive leaders

Staff spoke positively about the leadership of the service. One staff told us: “Management are supportive, if you have a problem. I would talk to the manager, [about concerns in care] I have raised something here and it was sorted out. I was listened to.”

We saw various compliments which had been received by the service which indicated the staff and leaders went above and beyond to provide high quality, compassionate care. Comments included: ‘If I can start with the last few weeks of my mother's life after she broke her femur. It would have been very easy (and justifiable) for you to have said that you were no longer able to look after her. This would have necessitated either a stay in hospital or a move to a nursing home, either of which would have been very traumatic for my mother. I am sure that she appreciated staying in surroundings that she knew well and with people that she also knew well.’ Another comment stated: ‘I just wanted to say thank you from Mum and I for inviting me to your Christmas Day celebrations. It made the day special for both of us (especially as there was no washing up involved!) and Mum was very pleased to get your sack of presents.' Processes were in place to enable leaders to manage the service effectively. For example, we saw that there were regular meetings in place for home managers under this provider where scenarios were discussed and any lessons learned. We saw that staff taking on leadership roles were provided with additional training and encouraged to complete external qualifications such as diplomas in health and social care.

Learning, improvement and innovation

There was a culture of continuous learning, improvement and innovation within the service. We saw that reflections took place following incidents and accidents, and actions were taken to minimise future risk. We saw staff received regular supervision and we saw staff meetings took place regularly. We saw actions were taken in response to staff surveys. We saw managers meetings took place which indicated an open and transparent culture. We saw certain scenarios were discussed in these meetings along with any lessons learnt. For example, we saw a recent scenario which happened at Stratton House was discussed with other managers and saw that lessons learned were sent to all seniors and nurses.

People were at the heart of the service and their feedback about the service shaped how this was ran. We saw evidence of this during our assessment, and feedback from staff and leaders echoed this. Staff told us: "People have a residents meeting, they are all good at speaking up. They would go to management, and if they ask us we would inform them who to speak with." There was a culture of continuous learning, improvement and innovation within the service. The manager explained how staff step outside of their role and about how staff are promoted within.
