How staff, teams and services work together

The practice had systems in place to work together for patients. The practice utilised tasks to follow up patients and recall them for reviews. Although this looked like there were a number of tasks outstanding, we found that this was to organise their work and not in relation to patient safety.

Monitoring and improving outcomes

Leaders told us monitoring and improving outcomes was important within their practice. However, it was difficult to see evidence of these discussions documented within meeting minutes and records. Staff told us of incidents and complaints which had led to improvements within the service, however the documentation of these did not fully evidence the action taken.

Patients care and treatment was being monitored by the practice, however there were some gaps with monitoring some patient's care and and treatment. This included some patients taking medicines which required monitoring.
