Assessing needs

Score: 3

Leaders and staff told us the practice would use a series of codes and alerts on the patient record to highlight people’s communication needs and any impairments. The practice had systems and processes in place to identify people’s needs and preferences during the registration process. Staff told us they checked people’s health, care, wellbeing and communication needs during health reviews.

A remote review of the patient record system showed that patients received appropriate long-term condition reviews. Patients with long-term conditions were offered an effective annual review to check their health and medicines needs were being met. For patients with the most complex needs, the GPs worked with other health and care professionals to deliver a coordinated package of care. Registers were kept of patients with different health requirements. Care coordinators managed the registers and invited patients to the practice for reviews and health checks according to their needs. We observed there was a wealth of health and social prescribing information for patients throughout the practice waiting area. Self-help advice tools were available for patients on the practice website. The practice identified patients with caring responsibilities and had a carers lead to support their needs. There was a carers policy that detailed how the practice identified carers which included patient information on notice boards, and as part of the New Patient Registration forms and Carer Registration forms. Pro-active identification of patients included periodic letters to patients or text messaging to encourage patients who had not yet been identified as a carer to make themselves known to the practice; and health professional identification where clinicians pro-actively identified carers as part of their patient consultations.
