Staff assessed the physical and mental health of all patients on admission. They developed individualised care plans which were reviewed regularly through multidisciplinary discussion and updated as needed. Care plans reflected patients’ assessed needs, and were personalised, holistic and recovery oriented. Staff provided a range of treatment and care for patients based on national guidance and best practice. They ensured that patients had good access to physical healthcare and supported them to live healthier lives. Staff used recognised rating scales to assess and record severity and outcomes.

Delivering evidence-based care and treatment

Score: 3

Patients we spoke with all felt their physical health was managed well by the hospital staff and they were confident about any treatment they were receiving for physical health problems. No patients we spoke with had a specialist care plan for nutrition and hydration. However, all patients were positive about the food quality, quantity, and choice.

We spoke with members of the multi-disciplinary team as part of the on-site activity, including occupational therapists and psychology. Staff were able to describe a varied activity timetable which worked towards a defined model of practice with standardised clinical assessment and treatment pathways. The service had access to a full range of specialists to meet the needs of the patients on the ward. Patients received care, treatment and support that was evidence-based and in line with good practice standards. Staff told us they complete a comprehensive multi-disciplinary assessment on admission and care plans and risk assessments are reviewed regularly. Patients had access to a full multi-disciplinary team including doctors, nurses, occupational therapists, and psychologists. Managers ensured staff had the right skills, qualifications, and experience to meet the needs of the patients in their care, including bank and agency staff. The provider had recently employed staff from overseas. Staff told us these new staff members were caring. However, they did not always have a good understanding of English, and this could affect communication. Managers gave each new member of staff a full induction to the service before they started work and supported staff through regular, constructive supervision and appraisal of their work. All staff we spoke with confirmed they had completed induction and had regular supervision.

The provider had processes in place to ensure patients received care, treatment and support that was evidence-based and in line with good practice standards. The ward staff worked well together as a multidisciplinary team and there was clear input from occupational therapy and psychology teams. All patients had their physical health assessed soon after admission and regularly reviewed during their time on the ward. Staff developed a comprehensive care plan for each patient that met their mental and physical health needs. Staff regularly reviewed and updated care plans when patients' needs changed. Care plans were personalised, holistic and recovery orientated. Patients and their family members were invited to attend ward round, family members could attend remotely if they were unable to attend in person due to long distance placements.
