Effective
People's immediate and ongoing needs were fully assessed. People were advised what to do if their condition got worse and where to seek further help and support. Medicine reviews for long term conditions were effective and detailed, patients were involved in their care and treatment and support was provided when necessary. Staff were supervised appropriately to ensure good clinical standards where monitored and maintained.

Assessing needs
Information we received was positive regarding patient's needs being met.
Staff told us they placed importance in providing individualistic care to patients to ensure they were providing effective care. The leaders were passionate about ensuring patients care was reviewed and updated regularly to provide high level care.
A remote review of the patient record system showed that patients received appropriate long-term condition reviews. Patients with long-term conditions were offered an effective annual review to check their health and medicines needs were being met. For patients with the most complex needs, the GPs worked with other health and care professionals to deliver a coordinated package of care. Registers were kept of patients with different health requirements. Care coordinators managed the registers and invited patients to the practice for reviews and health checks according to their needs. We observed there was a wealth of health and social prescribing information for patients throughout the practice waiting area. Self-help advice tools were available for patients on the practice website.
