Person-centred Care
Families told us they were involved in care planning, and one family member said, “I am not involved but I think they ask my [relative].”
A staff member said, “Person centred care is all about the person, their individual needs and wants.” Another said, “Everyone is different, so you need to do their care, not the same as another person’s care. Everyone has their own preferences to work within.”
We observed that staff knew people well, offered choices and respected peoples decisions. People were given choice of meals, drinks and activities. People were able to personalise their bedrooms. Care plans detailed peoples preferences.
Care provision, Integration and continuity
People experienced joined up care as the registered manager and care staff worked in partnership with district nurses and palliative care team.
The registered manager said, “we work collaboratively with the district nurses and palliative care team, we have worked well together for a number of years. We have made referrals to other teams such as the falls prevention team and dieticians, however our referrals have been turned down, we were told this was because we are a care home.”
A stakeholder said, “I have found Broadoaks brilliant, staff are fantastic, staff make you feel welcome. Staff always stay with me when I provide care for people. Staff are interested in learning. Staff know people well and their background.”
The registered manager had systems in place to identify changes in peoples health and made referrals to external professionals for support.

Planning for the future
The registered manager told us that they work with people who come for respite care to return home. The registered manager also said, “We aim to work with people and their families to plan for end of life care but often people are not open to planning ahead for this and prefer to make those decisions closer to when needed.”
