Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last inspection we rated this key question requires improvement. As this inspection the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The service had a proactive and positive culture of safety, based on openness and honesty. They listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. Monthly reviews of the service were undertaken, and discussions were held with staff in regular ‘toolbox talks’. The monthly newsletters sent to staff also included information about what the reviews had found and what action had been taken.

Safe systems, pathways and transitions

The service worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. Care plans contained detailed information about people’s needs and preferences. This was shared with professionals to support people’s continuing care needs and ensure people received continuity of care when transitioning between, for example, the home and hospital.

Safeguarding

The service worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. They concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The service shared concerns quickly and appropriately. Staff were knowledgeable about how to identify safeguarding concerns and were confident appropriate action would be taken in the event they raised concerns. When asked about safeguarding, a staff member told us, “[I’ve] completed safeguarding training. I'd speak to the staff member, manager, [name of provider], if I saw anything I didn't like.”

Involving people to manage risks

The service worked with people to understand and manage risks by thinking holistically. They provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Care plans contained detailed information which directed staff to consider and manage risks at each care delivery. For example, some people had fluctuating mobility needs and staff were encouraged to discuss with people how they were feeling and what support they needed at that time. The registered manager encouraged staff to promote independence and positive risk taking within safe parameters.

Safe environments

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Appropriate checks and audits were in place to ensure the premises and equipment were safe and risks were minimised.

Safe and effective staffing

Score: 3

The service made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. Systems were in place to ensure staff were recruited safely. New staff received an induction which did not end until both them and the registered manager were confident in their abilities. All staff spoken with told us they were very well supported.

Infection prevention and control

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Robust cleaning schedules were in place. Infection prevention and control audits took place regularly. Staff used personal protective equipment (PPE) appropriately and this was stored safely.

Medicines optimisation

The service made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. They involved people in planning, including when changes happened. People were administered their medicines as prescribed. Management of medicines followed good practice guidance. The provider was making improvements to people’s ‘as and when’ required medicines management. People were appropriately involved in decisions about their medicines.
