Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last inspection we rated this key question good. At this inspection the rating has remained the same. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People received assessments about their care needs which were based on their individual needs. Reviews of all aspects of their care took place regularly or when staff had identified people’s needs had changed.

Delivering evidence-based care and treatment

The service planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. The registered manager kept staff updated about good practice through review and discussions of policies. Staff told us information in care plans was relevant to the person. Staff led the reviews of people’s needs and dependency.

How staff, teams and services work together

The service worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff described a strong culture of teamwork. A staff member said, “[Staff] support equally, it’s all shared out.” Another told us, “We are very hands on, we all work as part of a team, [there are] very good relationships with all staff teams.”

Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support. Staff were proactive at providing people with information to prompt and encourage healthy lifestyle choices. The service was proactive at ensuring people had appropriate access to health appointments and regular visits from health professionals such as dentists and chiropodists. One relative, whose loved one had moved very recently to the home, told us, “I was talking about taking [name of relative] to the dentist, but they said not to worry, they have a dentist visit, so that’s helped.”

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Staff were encouraged to make suggestions about how people’s care and support could be improved. For example, some people had been reluctant to join ‘formalised’ activities at a table. Since activities had been undertaken in a less formal setting in the lounge, this person had started to participate in these activities.

The service told people about their rights around consent and respected these when delivering person-centred care and treatment. Care plans contained detailed information about how people had consented to their care. Staff described how they supported people to make choices about each aspect of their care. Where people could not consent to their care appropriate decision making was recorded.
