People had personalised care and support plans that were regularly updated to reflect their current needs. Care records were accessible to other health professionals enabling consistent care and support when transitioning between services. Staff understood the importance of promoting people’s independence. Staff knew about people’s capacity to make decisions through verbal or non-verbal means, and this was well documented. People were empowered to make their own decisions about their care and support.

Assessing needs

The registered manager completed an initial assessment of peoples needs to ensure the service remained relevant. A relative said, "Reviews are important. It's valuable being able to communicate with the service and ongoing reviews, which are excellent."

Staff told us they had access to up to date information to provide people with care and support to meet their individual needs. They told us any changes were discussed and records updated without delay.

Care plans included recorded reviews and updates to ensure staff and others had access to required information to meet people's changing needs.

Delivering evidence-based care and treatment

People received care and support tailored to their individual needs. People were supported to eat and drink and maintain a balanced diet. People lived in their own homes and were supported to enjoy healthy food and drink options.

Staff worked closely as small team to deliver person centred care. They worked with other health professionals to meet people's needs.

Records of daily care provided were regularly checked by the registered manager. There was evidence of very regular reviews and a focus on outcomes for people

How staff, teams and services work together

People had good support from both the service and their relatives to access other health services in support of their health and well being.

Where needed, staff liaised with health care professionals to ensure people's needs were met.

The service worked pro-actively to engage specialist support and advice to keep people safe and healthy.

Care plans were updated to record feedback and actions from other health professionals where their input was required.

Supporting people to live healthier lives

People and their relatives told us they were happy with the care and support provided and said that it enabled them to live as independently as possible in their own homes. A relative said, "[Person] has control. Myself and another relative support on his behalf because of his hearing problems."

Staff understood the importance of good communication and reviews with people's direct involvement which helped people to achieve good outcomes.

Records identified when people were at risk of poor nutrition and hydration. Clear guidance was given to staff on how to support people with their nutrition.

Monitoring and improving outcomes

People received a good service from caring staff who were attentive to their changing needs with a focus on achieving positive outcomes.

Staff discussed how they monitored people's well being to maximise their outcomes. A staff member said, "We monitor people's weights, one person was a diabetic we promoted balanced nutritious meals following a diabetic episode, and was then tested negative for diabetes."

The registered manager maintained regular contact with people who used the service to ensure they were satisfied with the care and support they were receiving. Where required changes following consultation were made ensuring the service remained relevant.

People and their relatives told us they were involved in their care and support and received information in a format they could understand and agree to. A relative said, "[The service] was discussed with him and signed. We try to give him as much control as possible."

Staff discussed how they supported people to express their views and be involved in decisions about their care and support. A staff member said, "We chat away to people at our visits, ask them how they are, if they are happy etc, all part of checking we are meeting their needs."

Care plans included signed confirmation of people's input, understanding and agreement to their care and support.
