We reviewed the systems, pathways and transitions within the service and how the service was involving people to manage risk. Systems worked well to ensure safe transition into and out of the service. We saw that staff included clients in their risk assessments, and they planned for clients unexpectedly leaving treatment. However, staff did not document that they followed the risk assessment formula in their handover documents. This meant that risks were not fully documented in handovers which could have led to incidents occurring.

All patients we spoke with were happy with the service and felt that it met their needs. Patients felt safe and described how the programme allowed them to progress through their treatment with positive results

Staff were knowledgeable about client risk.

We saw the service had processes in place to ensure safe transition into and out of the service.

Involving people to manage risks

People using the service told us they had a recovery plan and risk management plan in place. These plans were amended during the programme for patients as the patient improved and progressed through the programme.

Staff were knowledgable about client's risks.

Staff had handovers to ensure risk information was passed over and to help reduce adverse incidents. Their handover document included a framework for discussing risk to help this handover. However, Staff did not document their use of this framework and so there was no clear indication that risks had been discussed routinely. The provider had made some improvements to the handover document and associated handover processes in response to our findings at this assessment.
