Well-led
At the time of the assessment a service review was in progress which included governance and management. There were plans in place to improve the electronic patient record system to document the integrated palliative care outcome scale to help provide the best care. However, our review of risk management during our assessment identified some risks had not been documented and escalated appropriately.

Governance, management and sustainability
Staff told us they were clear about their roles and responsibilities. Managers and leaders were visible and approachable. Leaders told us the service review would support staff to meet patient needs going forward. Leaders provided monthly service updates to their division and attended governance and improvement meetings.
The service systems and processes were under review at the time of the assessment. There were clear roles, responsibilities and accountability in place for the nursing teams. The medical staffing had changed in the last 12 months and there was a current vacancy. There was no medical oversight of the service at the time of the assessment, but medical support was in place from the local hospice and hospital. The electronic patient record system was due to be updated to document the integrated palliative care outcomes. This would help give a better understanding of a patients symptoms and compare previous scores to adjust personal treatment plans. Digital records conformed to relevant digital and information security standards with arrangements in place for confidentiality of data management. There was no annual audit schedule in place at the time of the assessment. The trust told us this would be reviewed as part of their review of the service. There was a process to support and manage staff when their performance was poor or variable. We reviewed risk management as part of the assessment and found only one risk on the risk register relating to palliative care. The risk register and risk management was reviewed as part of the well-led assessment of the trust.
