Ratings - Acute wards for adults of working age and psychiatric intensive care units
Our view of the service
Date of Assessment: 10 September 2024 Nottinghamshire Healthcare NHS Foundation Trust’s acute wards for adults of working age and psychiatric intensive care units service consists of 8 wards at Highbury hospital and 4 wards at Sherwood Oaks hospital. During this assessment we carried out our onsite assessment at 2 of the 4 wards at Sherwood Oaks hospital. The service was last rated inadequate (March 2024). The report was published following CQC’s old inspection approach using key lines of enquiry (KLOEs), prompts and ratings characteristics. This assessment has been completed following the Care Quality Commission (CQC) new approach to assessment; Single Assessment Framework (SAF). During this assessment we visited Fir ward and Beech ward. This was an unannounced assessment, which means the provider was not told an assessment was going to be starting beforehand. During this assessment we looked at most quality statements across 1 key question and 1 quality statement across another key question: learning culture, safeguarding, involving people to manage risks, safe environments, safe and effective staffing, infection, prevention and control, medicines optimisation, person centred care. As we assessed enough quality statements for the key question of Safe, the rating for this key question reflects the findings of this assessment. However, as we did not assess quality statements from the key questions; Effective, Responsive, Caring and Well led, which means we use the ratings from the previous inspection to rate these key questions. During this assessment we found a breach in regulation under safe staffing and under good governance. We have asked the trust to submit an action plan outlining how they are going to meet these areas of concern.
People's experience of this service
During this assessment we spoke with 12 patients, 1 carer and reviewed 8 patient care and treatment plans and observed 2 activity sessions during the onsite assessment. Patients we spoke with felt safe on the wards and felt staff were supportive and kind. They told us the wards were clean, homely and welcoming. They felt involved in the development of their risk assessments and informed about discharge plans. However, patients told us they did not always have timely access to pain relief and one patient did not have access to their medication for 2 days, which people said led to their behaviours escalating. We saw staff were present, but interactions between staff and patients were infrequent. However, when there were interactions, they were pleasant and showed active listening.
