Stakeholders and people told us how they worked in partnership with the provider and how the provider received feedback, then created actions to be completed to improve for the future. The registered manager showed us how they invited the head of the local authorities quality team to analyse information received from hospitals following a hospital discharge, this was to work collaboratively with the local authority to improve on this process. The provider evidenced improvement plans and meeting minutes where meetings were held with the local authority. The registered manager was proactive in organising multi-disciplinary meetings to ensure all concerns were raised and peoples experience of care was safe and conducted in a quality standard.

Partnerships and communities

People and relatives told us staff assisted them in arranging support from other healthcare services as needed. People told us they felt listened to with the provider acting on information they shared. Where a concern had been shared with the provider, we saw that the provider would respond and meet with the person to resolve the issue. People told us how the provider contacted them regularly and would complete further checks such as home visits if required.

Partners told us how the provider was actively involved in information sharing meetings and where appropriate requested meetings to be held. The provider had engaged with the local authority to identify areas for improvement in response to a number of safeguarding alerts. The meeting presented actions for both the provider and local authority to work together to improve for the future.

We saw examples of the management team sharing safeguarding concerns with the local authority, safeguarding team and care quality commission. All concerns were shared in a timely manner and where an investigation was required this took place in a responsive time frame. The service was transparent and active in looking at ways to continually improve the service provided. Accidents and incidents were reported and recorded in line with the policies and procedures of the provider. This meant risks to people were further mitigated by the sharing of information about possible risks with other services for consideration and possible investigation.
