We found the service was providing well-led services. There were clear and effective governance, management and accountability arrangements and staff understood their role and responsibilities. We noted that any areas of concern highlighted during the assessment were immediately remedied.

Freedom to speak up

Feedback from staff indicated that they felt comfortable about speaking up. We were provided with some positive examples of staff ideas and suggestions being implemented.

The practice had a Freedom to Speak Up policy, and all staff had access to a Freedom to speak up guardian.

Governance, management and sustainability

Staff told us leaders were visible and approachable. Staff felt they were able to raise concerns without fear of retribution. They felt part of a strong team and supported by the clinicians and the management.

Individual patient care records, including clinical data, were written and managed securely and in line with current guidance and relevant legislation. Clinicians understood the requirements of legislation and guidance when considering consent and decision making, and we saw that consent was documented. However, we found some consent forms and patient records lacked specific information about the potential complications associated with the vasectomy surgery. The provider had already identified these concerns through their clinical record audits and had taken steps to address the findings. We saw that revised consent forms were in place and were appropriately completed.
