During an assessment under our new approach

Chatsworth Grange is a care home that provides personal and nursing care for up to 66 people. At the time of our inspection there were 58 people living in the home. The service provides support to younger and older adults, including people living with dementia. This inspection was prompted in part by receipt of information relating to incidents which occurred at the service. The information shared with CQC indicated potential concerns about the management of medication and standard of care. This assessment examined those risks. We did not find evidence to support the concerns raised. People received their medicines as prescribed. More details were needed in some records to fully understand how people communicated if they needed as required medicines and some improvement was needed for the recording of administration for some medications. This was actioned immediately by the registered manager. Staff recruitment procedures were not always robust around verifying references and ensuring gaps in employment were explained. Action was taken immediately to address this. Staff said they enjoyed working at the home and felt well supported by the registered manager. They were able to raise any issues with the management team and felt listened to. We observed positive interactions between people and staff. The principles of the Mental Capacity Act (MCA) were being followed. Where people had restrictions in place a best interest decision was recorded. Assessment activity started on 30 October 2024 and ended on 8 November 2024. We visited the service on 30 October 2024. We looked at all quality statements in safe and one quality statement in caring and responsive. We found some areas for development at this assessment but overall, the rating for this service remains the same.

During an inspection looking at part of the service

Chatsworth Grange care home provides care and accommodation for younger and older adults; including people living with dementia. The home can accommodate up to 66 people in one adapted building. The home is split into four separate units, across two floors. A secure garden surrounds the home. At the time of this inspection there were 56 people living in the home.

We found the following examples of good practice;

The service had created a visiting pod at the front of the building in January 2021 so residents could stay warm and indoors whilst having visitors. Relatives and other visitors would sit the other side of the glass windows and communicate via an intercom. We heard lots of ECGs visited regularly with the family.

On the day of our inspection the service’s last two staff members were in infection prevention control (IPC) local authority training. This was 15 minute bitesize sessions on the importance of correct use of face masks, hand hygiene, the one-hour chain of infection, the importance of correct laundry and waste management. All service staff could access these daily at midday and sessions were interactive with a live host who we heard made them fun and engaging. The RM, deputy and all the other staff had completed these sessions.

The service had a social network page with many photos and videos so residents and their relatives could be kept updated about activities and staff could see updated info and guidance. The RM told us livestreaming worked well and they had received positive feedback.

At the time of this inspection there were 62 people living in the home.

People’s experience of using this service and what we found

People were happy with the care they received. People felt safe and well-treated by staff. Staff knew about people's needs and preferences, and this led to people receiving personalised care. People were treated with dignity and respect, and their independence was promoted.

People living at Chatsworth Grange, their relatives and staff said they would recommend the home to family and friends. A relative commented, “I would definitely recommend it; book us in! Staff are really friendly. It’s like a home from home.”

Risks to people were assessed and minimised, to help them remain safe from avoidable harm. People were happy with the support they received with their medicines. We identified some minor improvements could be made to medicines management records.

People were supported by staff who received a range of training to ensure they had the right skills and experience to carry out their roles effectively. Staff supported people to maintain their health and they sought timely advice from health professionals, when needed.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests. The policies and systems in the service supported this practice.

People told us staff responded to them quickly and we found there were enough staff on each shift to meet people’s needs. People said they would not hesitate to raise any concerns with staff. We found complaints were appropriately managed.

The home was well-run, by an experienced registered manager. A range of regular checks were made on the quality of the care delivered, to ensure people received good quality care, in a comfortable environment. The home was clean, and people had access to facilities and any equipment they needed.

The last rating for this service was good (published 14 September 2018). Since this rating was awarded the registered provider of the service has changed. We have used the previous rating to inform our planning and decisions about the rating at this inspection.
