Assessing needs
We reviewed the practice’s results in the 2023 national GP patient survey. When looking at patients’ response to being involved in decisions about care and treatment, the results was better than the national average.
Reception staff were aware of the needs of the local community. Reception staff used signposting documents to refer patients to the appropriate healthcare professional. The practice participated in 3 audits that looked at whether appointments could have been seen by another health professional instead of a GP. Following the third audit, the practice had improved. Leaders were passionate about continuing to make improvements around care navigation and had signed up for the next phase of the ICB General Practice Improvement Programme (GPIP).
Clinical staff used templates when conducting care reviews to support the review of people’s wider health and wellbeing. Staff could refer people with social needs, such as those experiencing social isolation or housing difficulties, to a social prescriber.
