Well-led
We saw a clear governance structure and staff who understood their roles, responsibilities and systems of accountability. These were used to manage and deliver good quality care, treatment and support that was maintained. We saw that information received about risk, performance and outcomes were acted upon and changes were made to improve outcomes for patients. The practice focused on learning, innovation and improvement across their own organisation and also corroborated with the local provider community network and the wider integrated care board to achieve better health for patients.

Governance, management and sustainability
We saw there was a clear roles and responsibilities structure with systems of accountability and good governance. The provider was able to demonstrate how these were used to manage and deliver good quality, sustainable care, treatment and support. We saw how information about risk, performance and outcomes, was shared and how change was discussed and implemented following feedback from both staff and patients.
The practice had a stable, core team of GP partners with clear lead roles and responsibilities. The provider was open and transparent and completed a series of actions to ensure concerns highlighted during this assessment were promptly addressed. There was a quality improvement plan in place including for example, improvements in clinical and administration areas and better access for patients.

Learning, improvement and innovation
Interviews with staff and leaders demonstrated there were clear governance arrangements that supported staff to deliver high quality care and treatment that was tailored to the needs of the local population. Staff we spoke with were clear about their roles and responsibilities. Staff also told us how and to whom they would raise feedback, suggestions, and concerns. We saw there were regular meetings and learning events held monthly for the staff with clear feedback mechanisms to ensure all staff were kept up to date.
Policies, processes, and systems to support governance and management of the practice were managed by the GP partners and the management team. There were regular meetings during which issues significant to the delivery of clinical care were discussed and reviewed. There was an up-to-date business continuity plan in preparation for major incidents. We saw evidence of clinical audit, quality improvement, and engagement and learning from significant incidents.
