People were supported to live healthier lives. Staff and leaders supported people to make informed decisions about lifestyle choices. Health care professionals were involved in people’s care planning. This helped ensure people had access to the most up-to-date information about their health.

Supporting people to live healthier lives

Evidence from care plans and questionnaires, showed people had health care professionals involved in their care. People were encouraged by staff to follow a healthy lifestyle. Staff supported people to maintain their health for example by encouraging them to follow an exercise program designed by a physician.

The provider had processes in place to support people to live a healthy life. All their needs were assessed when a person started to use the service. The provider recorded goals and achievements in care plans. Health care professionals had input in people’s care plans and staff followed recommendations where these were in place. People and families were involved in reviewing care plans and making decisions about their care.

Staff told us they promoted healthy lifestyle for everyone they supported. Staff explained that they encouraged people to stay hydrated, eat a balanced diet and take some exercise. Staff shared some examples with us of how people had made some improvements to their health over a period of time. Care plans recorded some goals people had and things they wanted to achieve.
