Equity in experiences and outcomes

People received care and support that was tailored to their individual needs and preferences, and they were supported to achieve their gaols. One person told us how they were supported to attend clubs, go to church and maintain friendships with people outside of the service. Another person said, "Swimming is a goal. I’m gradually getting more confident."

Staff told us they knew people well, they felt they engaged effectively with them and provided person centred care. One member of staff told us, " Everyone is treated equally. We have keyworker meetings and things are highlighted, they have wonderful lives. We make sure people make decisions based on their wishes. We would never force people to do things, its always their choice. they do what they want to do, and they are much happier."

Care plans contained personal information, which recorded details about people and their lives. This information had been drawn together, where possible by the person, their family and staff. People’s communication needs were assessed and recorded in their care plans. Staff were aware of people’s communication needs and how to offer them support in ways they understood. There were systems and processes followed to consult with people, relatives, staff and healthcare professionals. Meetings were carried out, providing management with a mechanism for monitoring satisfaction with the service provided.
