The provider had appropriate governance, management and accountability arrangements. Staff understood their roles and responsibilities and there were systems in place to manage performance and risks to the quality of the service. Information and notifications were being submitted to external organisations as required. The provider and staff were working in partnership with other organisations, to support consistent care provision and joined-up care for people.

Governance, management and sustainability

Staff told us they understood their roles and responsibilities and the provider confirmed there were systems in place to manage performance and risks to the quality of the service. The provider said they had learned a lot whilst being under the ‘provider concerns’ and had made changes and improvements as a result. The provider told us they would make sure the service continued to be managed safely and effectively until the last person moved out.

We saw the provider had appropriate governance, management and accountability arrangements in place to make sure people living and working in the home remained safe and well supported. Information and notifications were being submitted as required and the provider continued to complete identified improvements, in accordance with their action plan.

Partnerships and communities

People benefitted from ‘joined-up working’ between themselves, the provider, the local authority and other care providers. This was particularly important with regard to making sure they could make fully informed choices about when and where they moved.

The provider told us how they were supporting people to move, due to the closure of the home. They said, “We are working to give the people here choice about where they are moving to and not to just take the first thing”. The provider reiterated what he had stated during a recent staff meeting; being that they would support and monitor people to ensure they had a safe and positive transition to their new placements. They would also maintain contact with the new placements, to answer any questions and help ensure people settled well. They also confirmed they would continue to work in partnership and collaboration with the local authority and other multidisciplinary teams.

The local authority confirmed the provider was working collaboratively with them, with regard to finding new accommodation for people to move to. They also confirmed that people were being supported to complete assessments with different care providers. Representatives from the local authority told us that everyone was working hard to make sure people’s new placements were successful and met people’s preferences, as well as their needs.

We saw consistent evidence that demonstrated how the provider and staff were working in partnership with other organisations, to support consistent care provision and joined-up care for people. People completed assessments with the local authority and other care providers, with regard to their transitions. The registered manager and the team supported people with viewing new properties and ensuring their voices were heard. The provider regularly sought feedback from people and their families. Families were also invited to people’s reviews and assessments and kept up to date with any issues. The provider attended different network forums, to improve working practices and ultimately provide better outcomes for people.
