Leaders had the skills, knowledge and experience to perform their roles. They had a good understanding of the service and were visible and approachable for patients and staff. Managers had effective and comprehensive governance arrangements in place to identify and monitor risk and had the relevant information to ensure the service was performing well. They were aware of the strengths and challenges within the service. Where required lessons had been learned and communicated from incidents, which were used to drive improvement. Managers ensured staff were appropriately trained and supervised and promoted a positive and open culture.

Shared direction and culture

Staff were aware of the vision, values and strategy of the provider and the service. Staff told us they thought there was a positive, open culture and they felt respected and valued. They said they worked well together as a team. Staff worked across the wards when they needed to, so silos had not formed.

The provider’s values were displayed across the site and were embedded within recruitment, policies and procedures. ‘Safewards’ had been re-introduced across the wards earlier in 2024 and managers were in the process of getting it fully embedded. Staff had a good understanding of the model and could explain the techniques used and how it would help patients. The provider used staff engagement surveys to gather feedback from staff and had subsequently implemented action plans showing areas they were going to prioritise as an organisation. Overall, the latest results from 2024 showed that staff were positive about working at Lakeside View and for the provider. Staff wellbeing occasions took place across the year such as coffee mornings and seasonal events.

Capable, compassionate and inclusive leaders

Managers said there was stable leadership across the wards and a positive culture had developed within the teams. Staff told us that leaders were supportive, visible and approachable. Staff told us that they had been given opportunities to develop and progress in their career and could attend leadership training.

Managers encouraged staff to make improvements to the service and several staff were leading on quality improvement projects. Managerial presence was available in the hospital across the 24-hour period.

Freedom to speak up

Staff knew how to raise a concern or complaint and felt confident to do so. Staff felt that there was a positive and open culture.

The provider had a whistleblowing policy and procedure in place. Staff had access to a Freedom to Speak up guardian where they could raise concerns. Managers told us nothing specific had been raised.

Governance, management and sustainability

Managers had efficient and comprehensive governance arrangements in place and had the relevant information to ensure the service was performing well. They were aware of the strengths and challenges within the service. Managers attended regular clinical governance meetings and had sufficient oversight from senior leaders within the organisation. Information was cascaded to staff through team meetings.

We reviewed monthly governance meeting and patient safety meeting minutes. These were attended by a range of staff. Key performance indicators and monthly audit processes helped give sufficient oversight of the service. When required, appropriate actions were put in place with agreed deadlines for completion. The provider had a risk register which clearly stated what the risk was and what mitigation had been put in place to help reduce it. These risks were discussed within governance meetings alongside other items such as the site improvement plan, patient and carer feedback, stakeholder feedback, incidents, environmental and infection prevention control, staffing and safeguarding. When required appropriate actions were put in place and reviewed at the next meeting.
