We assessed a total of 3 quality statements from this key question. We have combined the scores for these areas with scores based on the rating from the last inspection, which was good. Our rating for this key question remains good. We found safety was a top priority, and staff took all concerns seriously. When things went wrong, staff acted to ensure people remained safe. Managers investigated all reported incidents to reduce the likelihood of them happening again. The management team ensured the environment was safe and all equipment was maintained to support the safe delivery of care. Staff assessed and managed the risk of infection and mitigated the risk of any infection spreading by maintaining a clean environment and liaising with appropriate agencies promptly.

Learning culture

People told us the staff and the leadership team were open to improvement and willing to learn from concerns. One relative told us, “Any issues here are dealt with by [the registered manager] immediately and we are kept informed at all times”. And, “The manager, deputies and staff are open and transparent, nothing is ever hidden, there’s no hidden agendas, and I feel I can discuss any concerns I may have with them at any point”.

The register manager encouraged a culture of openness and took seriously any concerns raised. One staff member told us, “I have no concerns about the safety of our residents and our staff, the manager and deputies are very open and we can always go to them to talk about any concerns we might have. They [the management team] will listen and take action.” Another staff member said, “[The registered manager] is always available, even if it’s at the end of the phone to make sure we feel confident in keeping people safe and are following the correct processes.”

We reviewed multiple documents in relation to audits, checks and action taken. Daily meetings with staff were used to discuss any incidents which had occurred and to ensure action was taken to make changes to peoples care plans and risk assessments. Incident analysis was completed to find the cause of incidents and make changes to prevent reoccurrence. This meant staff were continuously learning when things went wrong and adjustments were made to the service people received. There was a commitment from staff and the management team to learn from mistakes and make improvements to ensure people received safe and effective care.

Safe environments

People and their relatives were happy with the environment and felt the home was well maintained and safe. One person told us, "The home is great, staff are lovely and I feel safe”.

The registered manager told us about the refurbishment plans for bedrooms and the purchasing of new furniture. They described the checks in place to ensure the environment and equipment were well maintained. Staff explained the checks they completed to ensure the environment and equipment were well maintained. One staff member told us, "We check each room a number of times a day. If we find an issue, it is flagged and repaired quickly."

The home was well maintained and kept clean. We saw checks were being carried out on equipment and cleaning was undertaken. For example, we saw hoists being cleaned in between use.

The registered manager had a range of effective systems to ensure the home was in a good state of repair. There were regular checks on equipment and the environment, which ensured any issues were identified and actioned to keep people safe.

Infection prevention and control

People and their relatives described the home as clean and well maintained and reported staff used PPE and washed their hands.

The registered manager told us they carried out visual checks daily on the cleanliness of the home. Staff told us they received appropriate training in infection prevention and control, and we saw evidence of this. The staff told us they had access to personal protective equipment (PPE) and followed appropriate infection control guidance.

Our observations confirmed what people and staff told us about infection prevention and control practice. We found the environment to be clean and malodour free. There were housekeeping staff around throughout the day. Bathrooms and toilets were clean and fresh. We observed staff completing cleaning schedules during our on-site assessment.

Effective systems, such as audits and an infection prevention and control policy ensured the risk of preventable infection was reduced.
