Assessing needs

Score: 3

People were unable to recall if they had been involved in an assessment of their needs and the development and review of their care plans and risk assessments. We received mixed feedback about how well staff meet people’s individual care and support needs. Four people raised concerns about their morning routines and not being able to have a lie in the morning or go to bed before 7pm. However, 1 person told us they had discussed this with the registered manager who took action. People also told us there were limited activities available, including opportunities to access the community and garden. A person said, “I seem to get what I need.” Another person said, “It’s just general care, not particularly based on what I need.”

Staff demonstrated a good awareness of people’s individual care and support needs, including people’s preferences and routines. Staff confirmed care plans and risk assessments were kept up to date and reflected people’s current needs. Staff were confident that changes to people’s health and well-being were quickly responded to. Staff and the registered manager confirmed that, on the whole, the staff team were a consistent, stable and experienced team. A staff member said, “Care plans and risk assessments are much more detailed than they were a few years ago and are kept up to date. They are clearly set out and easy to follow.” The registered manager said face to face pre-assessments were completed and this information was shared with staff to determine if the person’s needs could be met or if there were any additional training or resources needed. Staff confirmed this was correct. The registered manager also said care plans and risk assessment were developed, and reviewed monthly by the team leaders. Changes to records were completed by the registered manager.

The provider had a pre-assessment process. There was also a process of how care plans and risk assessments were to be developed and reviewed. This was to ensure staff had continued up to date guidance. We saw that care plans and risk assessments were up to date, detailed and reflective of peoples’ current needs. Guidance was person centred; preferences, routines were recorded and respected. There was good use of cross referencing to other care plans and/ risk assessments. Communication needs were assessed and planned for. For supplementary records, there was some inconsistences in the level of information recorded around food intake. This was raised with the registered manager as there was no clear formal process of monitoring in place.
