Assessing needs

Score: 3

People told us they had a care plan. Most people said this was kept up to date and they were involved in reviewing it, although they hadn’t read the document itself. One person said, “I have a folder that’s all out of date, things are done on the app, but I haven’t got a clue about that.” Another person said, “I have a care plan in place, but I’ve never read it, the carers always do everything they need to for me.” A relative said, “We have a care plan, and it gets reviewed from time to time.” “I’m happy with what’s contained in the plan.” Another relative told us how the registered manager had visited them at home to assess [relative] care needs and agree a plan. This relative also said, “They kept to the times agreed and the continuity was outstanding.” “This alone helped a very private couple come to terms with the need of care in the home.”

We saw that people’s care plans included assessments of need, as well as risk assessments. These covered areas that were important to people, such as health, daily living, and goals to be achieved. All of these were regularly reviewed and updated as people’s needs changed. Frequent contact with people was also made by office staff to help ensure they were receiving the care and support they required.
