People were provided with person centred care involving others that was based on their preferences. The provider helped ensure people had good continuity of care where they moved between services. People were supported to have good end of life care and their decisions were respected.

Person-centred Care

People told us they were happy living in the home. People were in control of their care and support and were respected and valued as individuals. People told us about how they were involved in decision about the support and care they received. One person told us, “We discussed my condition when I first came here and they wrote it down” Another said, “We have discussed what I need and they do seem to listen to my preferences”. Where people had a preference for male or female staff, this was respected. One person told us how female staff would attend to their personal care.

Leaders were clear in explaining to staff what they expected from them while supporting people. Staff we spoke with told us people's care plans were personalised and provided them with the information they needed to know to support people in the way they preferred.

We saw that people received appropriate care and treatment. This included staff making adjustments to meet people's needs. Staff told us, "We let people do what they can but we help where it is needed. For example, having a routine that makes the person feel at ease."

Care provision, Integration and continuity

Staff told us they supported people to access healthcare such as from community nurses, GP and other health professional such as PEG feed. Whatever people’s needs the staff team ensured appropriate referrals were made such as, speech and language therapist (SALT), Tissue viability specialists, GPs, and diabetic specialist, and other treatment such as for specific health conditions. Staff worked well with these professionals and ensure guidance was adhered to.

People told us that they received the support they needed to access health and care services to ensure their needs were met.

Procesess were in place to ensure people's needs were assessed and action taken where necessary, so they received the correct treatment and care.

Planning for the future

Score: 3

People's care plans contained relevant information about religious beliefs, sexuality and what was important to the person for end-of-life care. People's preferences were respected. For example, one person did not want repositioning when they were in bed. Staff had explained the risks to them in a way they understood and had respected their wishes.

A member of the management team told us, “We have a good relationship with advance nurse practitioners regarding end-of-life care. I support staff with end-of-life training and we use the local palliative care nurse team and also MacMillan nurses. We are working well with a local hospice where people want to spend their final days in a care home. We always have a conversation with new admissions or their relatives and involve people as much as practicable in palliative care reviews. We liaise with the multi disciplinary team so we can identify what works well and where changes to care are needed. "

Processes were in place to ensure that all staff were aware of people's preferences and wishes. People and where appropriate their families were consulted about their end of life wishes.
