Supporting people to live healthier lives

Relatives felt staff supported people to access healthcare and other personal care services. One family member told us “[My relative’s] hair is done every two weeks, and a chiropodist comes”.

A member of the management team told us people have recently changed GP at the request of the GP surgery. They told us this had impacted on people’s health care. For example, medication reviews had not been completed. They told us they had been in regular contact with the new GP surgery to arrange medicines reviews to ensure people’s medicines were always effective. Staff told us how they worked closely with the local district nursing and acute response teams when people were unwell. Staff from these teams visited regularly to assess people’s needs and provide treatment. Staff explained how they acted on advice given and contact the teams again if people’s health did not improve.

People were supported and received treatment from professionals they trusted such as opticians, dentists and podiatrists. Where people did not have a preferred professional, staff arranged this for them. Staff contacted the home visiting team or acute response team if people needed medical treatment at the service. When requested staff supported a paramedic practitioner to complete a virtual ward round.
