Staff assessed people’s needs and we found these were reviewed regularly, following changes in needs, any incidents and hospital discharges, to ensure the best possible outcomes for their health and well-being. Effective systems were found to be in place to communicate any changes to people’s needs between staff members. We found staff supported people to access other health and social care services when needed, so people could live healthier lives. Throughout the site visit, the inspector observed staff supporting people in the least restrictive ways, asked for people’s consent and offered choices.

Assessing needs

People and their relatives told us their needs were assessed before they moved in to the home and documented in their care plans, so they received care according to their preferences. One relative told us, "I can talk to any of the staff and discuss my relative's care."

Staff and managers knew the process around assessing people’s needs before they moved in to the home. One staff member explained, "We complete pre-admission assessments so we know people's needs and preferences." Members of staff told us that changes in people's needs were communicated well throughout the team and reviews of people’s care needs took place regularly.

People’s needs were assessed before moving into the home. The information gathered at the assessment then formed the basis for people's care plans. Care plans demonstrated that risks to people were identified and also documented their health, care, well-being and communication needs. At the time of our assessment, we observed people, their family members and other health and social care professionals taking part in care reviews. Some people within the home had experienced delays with care reviews, but this had improved with the recruitment of senior staff.

Supporting people to live healthier lives

People and their relatives told us they could access healthcare services when needed and staff supported them to do so. One person told us how the GP visited the home on a weekly basis. People were able to access the GP if necessary. A relative told us the home had good links with the local continence service and had worked to resolve and improve their relative's outcomes in this area.

Management and staff knew which healthcare services they could access on behalf of people and how to do so effectively. The Manager explained the links the service had made with different health and social care professionals, including; GP’s, Dentists, District Nurses, Occupational Therapists, Physiotherapists, Tissue Viability Nurses and Chiropodists.

Care plans evidenced input and advice from other healthcare professional, including Tissue Viability Nurses (TVN) and the Speech and Language Therapy team (SaLT). People were supported to access health and social care services. Professionals working with the home shared positive feedback about how well the service communicated.
