Effective
Patients received effective care and treatment that met their needs. Patients’ needs were assessed and regularly monitored. National data for the number of women screened for cervical cancer and for 1 of 5 childhood immunisations were below the World Health Organization (WHO) target however the provider had a plan to address this. The practice had systems and processes to keep clinicians up to date with current evidence-based practice. We found that patients’ immediate and ongoing needs were fully assessed, these included interventions required to ensure effective care was delivered.

Assessing needs
Patient feedback about their experience of assessing needs at the practice was positive. People told us they felt involved in the assessment of their needs and decisions about their care. People were confident that their individual needs had been appropriately assessed and that they received an explanation in a way they could understand. The GP patient survey data was positive regarding people having confidence and trust in the healthcare professional, being listened to by them and being treated with care and concern. Feedback from 298 patients who completed the Friends and Family Test between April 2023 and March 2024 was mostly positive. Of those who participated, 270 (91%) said they would be likely or extremely likely to recommend the service. Neutral responses were received from 13 patients (4%) and 15 negatively (5%). Comments from patients included that the clinician had given clear advice and guidance, displayed understanding of the issues and they felt they had a thorough assessment. The practice had reviewed and responded to feedback received and was taking action to make improvements when required.
We spoke with leaders and staff about how patients’ needs were assessed. They demonstrated how information was used and shared to ensure high quality care was provided. They gave examples of how patients’ individual needs were met and managed effectively. Staff told us how the service had been impacted by reduced staffing in the nursing team however we saw improvements with this at the time of the assessment. The practice worked closely with the Primary Care Network (PCN) to improve health inequalities.
The provider had systems and processes to keep clinicians up to date with current evidence-based practice. The provider had a system in place to ensure patients’ assessments were up to date and that action was taken to follow up any action required. People were invited for appropriate health checks. The service was below the 80% uptake for cervical screening target with 76.8% uptake. Initiatives were in place to help increase uptake, for example, providing information to patients about the benefits of screening and appointments with clinicians available to pre book and attend at short notice. The service had met the 95% World Health Organization (WHO) target for childhood vaccination in 4 of the 5 target areas. The 1 area below this target was slightly below the 90% expected minimum at 89.2% however the provider had a plan to address this. The provider had systems to identify patients' communication needs and preferences in addition to caring responsibilities. They had access to an interpretation service and information was available in a variety of formats to meet patients’ needs.
