Learning culture

Regulations met

At our previous inspection we identified a serious incident that had not been reported or investigated appropriately. During this inspection we noted the following improvements: We were provided with a comprehensive ‘After Action Review’ of the incident concerned. This provided a detailed analysis of the event, including what had happened, what could be expected and identified key areas of learning. The review had been undertaken by a number of senior managers within CNWL and included a vascular surgeon from Milton Keynes Hospital to ensure that all aspects of the incident were considered. As a result of this review, a decision had been made to implement datix reporting for all unplanned patient admissions to hospital. We reviewed 6 recent hospital admissions and found corresponding datix reports had been completed. All staff we spoke with were aware of this new policy and confirmed it was being implemented successfully.
