Feedback from staff identified a culture of mistrust and a perception of poor communication which we were told was affecting morale and staff retention.

Shared direction and culture

During the assessment process we spoke with a number of staff in the ward area and held a focus group. Staff we spoke with identified increased support locally from the unit manager. However, all staff felt there was a disconnect between them and any more senior managers above band 7. Staff expressed a culture of mistrust which affected moral and raised concerns that decisions affecting them and patient safety would be made without there input. Although the ward team were extremely complementary of each other describing 'a friendly and close team'. One staff member reported working 'in a few trusts with this one being the most cohesive'.

The trust provided us with evidence of a comprehensive package of support which included opportunity for monthly off site/ward meetings. Support from a lead nurse working clinically, increased preceptorship support and a number of staff development opportunities. Staff we spoke with did not discuss these improvements.

Freedom to speak up

Score: 3

Staff we spoke with did not appear to have confidence in the Freedom to Speak Up process (FTSU). Staff felt they were not supported apart from the ward manager who was a positive leader who would advocate for them. Staff had cancelled a meeting provided by the trust with the FTSU to discuss concerns relating to staffing and morale. As part of this assessment we held a focus group that was attended by 10 staff. Positive changes were recognised however, the underlying mistrust of senior managers appeared to affect the staff morale and overall culture of the service. It appears that staff are worried that the positive changes that have happened will not continue. For example, reduction in beds whilst evidence provided by the trust supported that staff were heard and were committed to positive recruitment and patient safety.
