About the service

Best Care JS Limited is a domiciliary care agency providing personal care to four people living in their own homes at the time of inspection.

People’s experience of using this service and what we found

People and relatives told us they were happy with the care and support provided. A relative said, “To be honest, it’s fantastic. We are very pleased.”

Systems were in place to help protect people from the risk of abuse. Risk assessments were in place which included information about how to mitigate risks people faced. Infection control and prevention measures had been put in place. There were enough staff working at the service to support people in a safe way, and pre employment checks were carried out on staff to check their suitability. Accidents and incidents were reviewed to reduce the risk of similar incidents re-occurring.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

Assessments were carried out before the provision of care to determine what people needed support with and if the service was able to meet those needs. The service worked with other agencies to promote people’s health, safety and wellbeing. People were supported to eat a healthy diet. Staff had undertaken training to help them develop knowledge and skills relevant to their role. People were able to make decisions about their care.

Quality assurance and monitoring systems were in place to help drive improvements at the service. Some of these included seeking the views of people who used the service and others. The service had links with other agencies to help develop best practice. There was a clear management structure in place and staff spoke positively about the support they received.

Rating at last inspection and update

The last rating for this service was requires improvement (published 26 January 2021). At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

Why we inspected

We carried out an announced comprehensive inspection of this service on 21 and 27 January 2020. Breaches of legal requirements were found. The provider completed an action plan after the last inspection to show what they would do and by when to improve safe care and treatment, good governance and fit and proper persons employed.

We undertook this focused inspection to check they had followed their action plan and to confirm they now met legal requirements. This report only covers our findings in relation to the Key Questions safe and well-led which contain those requirements. The ratings from the previous comprehensive inspection for those key questions not looked at on this occasion were used in calculating the overall rating at this inspection. The overall rating for the service has changed from requires improvement to good. This is based on the findings at this inspection.

About the service

Best Care JS Limited is a domiciliary care agency providing support with personal care to people living in their own homes. Four people were using the service at the time of inspection.

People’s experience of using this service and what we found

Risk assessments were in place, which included information about how to support people safely. Robust checks were carried out on prospective staff to help ensure only suitable people were employed to work at the service. There were enough staff to meet people’s needs and people told us staff were punctual.

Assessments were carried out of people’s needs prior to the provision of care to determine what they wanted support with. People and their relatives were involved in these assessments.

Effective systems were in place to monitor and improve the quality and safety of support provided to people.

Rating at last inspection

The last rating for this service was requires improvement (published 20 March 2020) and there were multiple breaches of regulation. The provider completed an action plan after the last inspection to show what they would do and by when to improve. At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

Why we inspected

We undertook this inspection to check whether the provider had met the requirements in relation to Regulation 9 (Person-centred care), Regulation 12 (Safe care and treatment), Regulation 17 (Good governance) and Regulation 19 (Fit and proper persons) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. The overall rating for the service has not changed following this targeted inspection and remains requires improvement.

Targeted inspections do not look at an entire key question, only the part of the key question we are specifically concerned about. Targeted inspections do not change the rating from the previous inspection. This is because they do not assess all areas of a key question.

We will continue to monitor information we receive about the service until we return to visit as per our re-inspection programme. If we receive any concerning information we may inspect sooner.

About the service

Best Care JS Limited is a domiciliary care agency and is based in the London Borough of Barking & Dagenham. The service provides personal care to adults in their own homes. Two people were using the service at the time of the inspection. Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do, we also consider any wider social care provided.

People’s experience of using this service

Risk assessments were not completed to ensure people were safe at all times. Assessments of needs were not completed before people started using the service to decide if the service was suitable to effectively meet people's needs.

Staff recruitment processes were not robust to ensure all staff were appropriately checked and were suitable to provide safe care. The service had enough staff. However, staff did not always have supervision to discuss their practice and training needs. We made a recommendation in this area.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

Staff were caring and people's privacy and dignity was respected.

Staff understood equality and diversity and how to treat people without discrimination.

There were systems in place for infection control, recording, incidents, managing complaints and drawing lessons when things went wrong.

The service developed systems for obtaining feedback from people and other stakeholders.

This was a planned inspection based on when the service registered with the CQC.

Enforcement

We have identified breaches in relation to safe care and treatment, person-centred care and good governance. You can see what action we have asked the provider to take at the end of this full report.

Follow up

We will request an action plan for the provider to understand what they will do to improve the standards of quality and safety. We will work alongside the provider and local authority to monitor progress. We will return to visit as per our re-inspection programme. If we receive any concerning information we may inspect sooner.
