During an inspection looking at part of the service

Following our previous inspection on 14 January 2020, the practice was rated Good overall and for safe, effective, caring and responsive, but requires improvement for providing well led services.

We carried out an announced review at Park Road Group Practice (also known as The Elms Medical Centre) on 26 & 39 July 2021. Overall, the practice is rated as Good.

Why we carried out this review

This review was a follow-up of information without undertaking a site visit inspection to assess the key question:

  • Well-led.

We reviewed the breach identified at the last inspection of Regulation 17 HSCA (RA) Regulations 2014 Good governance. The regulation was not being met because:

  • A comprehensive system was not in place for ensuring emergency drugs and equipment met requirements.

We also reviewed the areas where the previous inspection identified that the provider should make improvements by:

  • Reviewing clinical rooms to ensure facilities comply with best practice guidelines.
  • Reviewing recruitment files to ensure all necessary documentation is included.

How we carried out the review

Throughout the pandemic CQC has continued to regulate and respond to risk. However, taking into account the circumstances arising as a result of the pandemic, and in order to reduce risk, we have conducted our reviews differently.

This included:

  • Reviewing action plans sent to us by the provider
  • Requesting evidence from the provider
  • Speaking with staff using video conferencing

Our findings

We based our judgement of the quality of care at this service on a combination of:

  • information from our ongoing monitoring of data about services and
  • information from the provider, patients, the public and other organisations.

We did not change the overall rating for this service following our review.

We found that:

  • The breach of Regulation 17 Good governance had been addressed and a comprehensive system was now in place for ensuring emergency drugs and equipment met requirements.
  • Clinical rooms facilities now complied with best practice guidelines.
  • Recruitment files had been reviewed and audited to ensure all necessary documentation was included.

Whilst we found no breaches of regulations, the provider should:

  • Ensure the emergency drugs protocol wording reflects the appropriate action to be taken.
  • Ensure statutory notifications are submitted in a timely manner.
  • Continue with the application to register an additional Registered Manager in the absence of the current registered manager.

During an inspection looking at part of the service

We carried out an announced focussed inspection at Park Road Group Practice on 14 January 2020 as part of our inspection programme. The Practice is also known as The Elms Medical Centre.

We carried out an inspection of this service due to the length of time since the last inspection. Following our Annual Regulatory Review of the information available to us, including information provided by the practice, we planned to focus our inspection on the following key questions:

  • Effective
  • Well Led

From the Annual Regulatory Review we carried forward the ratings from the last comprehensive inspection for the following key questions: Safe, Caring and Responsive.

We based our judgement of the quality of care at this service on a combination of:

  • what we found when we inspected
  • information from our ongoing monitoring of data about services and
  • information from the provider, patients, the public and other organisations.

We received 45 CQC feedback cards given to patients before and during the inspection. Overall, the comments made by patients were positive about the services provided and the practice staff.

We have rated this practice as good overall. We have rated all population groups as good.

We rated the practice as requires improvement for well led because:

  • The system for reviewing emergency medicines and equipment was not robust.

We rated the practice as good for providing effective services because:

  • Staff worked together and with other organisations to deliver effective care and treatment.
  • Patients’ needs were assessed, and care and treatment was delivered in line with current legislation, standards and evidence-based guidance.
  • Staff told us they felt well supported and had access to training to support them in their roles.

The area where the provider must make improvements:

  • Establish effective systems and processes to ensure good governance in accordance with the fundamental standards of care

During a routine inspection

We carried out an announced comprehensive inspection at this practice on the 15th October 2014 and at this time the practice was rated as good.

However, breaches of a legal requirement were also found. After the comprehensive inspection the practice wrote to us to say what they would do to meet the following legal requirements set out in the Health and Social Care Act (HSCA) 2008:

Regulation 21 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010, which corresponds to Regulation 19 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

On the 21st June 2016 we carried out a focused review of this service under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. This review was carried out to check whether the provider had completed the improvements identified during the comprehensive inspection carried out in October 2014.

The findings of this review were as follows:

  • The practice had addressed the issues identified during the previous inspection.

  • Appropriate recruitment checks had been carried out for staff and the practice had updated their recruitment policy to include all required checks for newly employed staff. The practice had undertaken Disclosure and Barring Service (DBS) checks for staff members.

Letter from the Chief Inspector of General Practice

This is the report of findings from our inspection of Park Road Group Practice. Park Road Group Practice is also known as The Elms Medical Centre and is registered with the Care Quality Commission to provide primary care services.

We undertook a planned, comprehensive inspection on 15 October 2014 at the practice location at the Elms Medical Centre, Dingle, Liverpool. We spoke with patients, relatives, staff and the practice management team.

The practice was rated as Good. They provided safe, effective, responsive and compassionate care that was well led and addressed the needs of the diverse population it served. However there were aspects of the service that needed to improve.

Our key findings were as follows:

  • There were aspects of the service that needed improvement. There were systems in place to ensure safe patient care and that lessons were learnt from adverse events and incidents. The premises were clean and patients were protected from the risk of infection. Systems were in place to ensure medicines were appropriately stored and used. However improvements were required to ensure staff were safely recruited and required information was held in relation to staff.         
  • The practice was caring. Patients spoke highly of the practice. They said they were always treated with dignity and respect, listened to and staff were kind, compassionate and caring.
  • The practice was responsive. The practice served patients in a deprived area of Liverpool. The practice provided good care to its population taking into account their cultural, religious, socio economic and language needs. The appointment system had been improved to enable good access to the patient population.
  • The practice was effective. Patient’s needs were assessed and care was planned and delivered in line with current guidance and legislation. The practice promoted health education to empower patients to live healthier lives.
  • The practice was well led. The practice worked hard to monitor, evaluate and improve services. They worked in collaboration with other practices and the Clinical Commissioning Group (CCG) within the Neighbourhood Team. Staff enjoyed working for the practice and felt well supported and valued.

There were areas of practice where the provider needs to make improvements. 

Importantly, the provider must:

  • Take action to ensure its recruitment arrangements are in line with Schedule 3 of the Health and Social Care Act 2008 to ensure necessary employment checks are in place for all staff and these checks are relevant to the roles.

In addition the provider should:

  • Follow up audits and undertake re audits to ensure the audit cycle is complete and improvements can be demonstrated. Review of actions taken following complaints should also be undertaken to demonstrate learning and improvements.
  • Review and update fire risk assessments and the fire precautions policy and procedures to reflect the local situation and current guidance. Fire alarm checks should be recorded fully to ensure they demonstrate these have been undertaken appropriately.
  • Risk assessments and risk management procedures should be reviewed to localise and update in order to reflect current guidance and legislation.
