Brookdale Surgery is the registered provider and provides primary care services to its registered list of 2300 patients. It is part of and managed by gtd healthcare, a not for profit social enterprise, and is overseen by Manchester Health and Care Commissioning.
The practice is situated in an area at number one on the deprivation scale (the lower the number, the higher the deprivation). People living in more deprived areas tend to have greater need for health services.
The practice has an Alternative Primary Medical Services (APMS) contract. The APMS contract is the contract between general practices and NHS England for delivering primary care services to local communities.
gtd healthcare took over the practice in May 2018. An intense safety and support strategy for both clinical and non-clinical aspects of the practice was commenced. This is still ongoing and will be for the foreseeable future to ensure the safety of all patients.
The practice is registered with the Care Quality Commission (CQC) to provide the regulated activities of diagnostic and screening procedures; maternity and midwifery services and treatment of disease, disorder and injury.
