During a routine inspection

We carried out this announced inspection on 21 November 2017 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered provider was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a CQC inspector who was supported by a specialist dental adviser.

We told the NHS England area team that we were inspecting the practice. They had no information of concern.

To get to the heart of patients’ experiences of care and treatment, we always ask the following five questions:

• Is it safe?

• Is it effective?

• Is it caring?

• Is it responsive to people’s needs?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

Are services safe?

We found that this practice was providing safe care in accordance with the relevant regulations.

Are services effective?

We found that this practice was providing effective care in accordance with the relevant regulations.

Are services caring?

We found that this practice was providing caring services in accordance with the relevant regulations.

Are services responsive?

We found that this practice was providing responsive care in accordance with the relevant regulations.

Are services well-led?

We found that this practice was providing well-led care in accordance with the relevant regulations.

Background

24/7 Dental is located in the London borough of Harrow and provides private treatment to patients of all ages.

There is access for people who use wheelchairs and those with pushchairs. Car parking spaces are available near the practice.

Practice staffing consists of a principal dentist, four dentists, four dental nurses, one trainee dental nurse, one receptionist and a practice manager.

The practice is owned by an individual who is the principal dentist there. They have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated regulations about how the practice is run.

The practice is open Monday to Sunday 8am to 10pm

The practice facilities include two treatment rooms a decontamination room, X-ray room, reception/waiting area and a staff room.

On the day of inspection we collected feedback from 45 patients. This information gave us a positive view of the practice.

During the inspection we spoke with the principal dentist, one dentist, one dental nurse and the Practice manager. We looked at practice policies and procedures and other records about how the service is managed.

Our key findings were:

  • The practice was clean and well maintained.
  • The practice had infection control procedures which reflected published guidance.
  • Staff knew how to deal with emergencies. Appropriate medicines and life-saving equipment were available.
  • The practice had systems to help them manage risk.
  • The practice had suitable safeguarding processes and staff knew their responsibilities for safeguarding adults and children.
  • The practice had thorough staff recruitment procedures.
  • The clinical staff provided patients’ care and treatment in line with current guidelines.
  • Staff treated patients with dignity and respect and took care to protect their privacy and personal information.
  • The appointment system met patients’ needs.
  • The practice had effective leadership. Staff felt involved and supported and worked well as a team.
  • The practice asked staff and patients for feedback about the services they provided.
  • The practice dealt with complaints positively and efficiently.

11 December 2013

During an inspection looking at part of the service

We carried out an inspection following compliance actions that we made at the time of our previous inspection on the 5 August 2013.

During that inspection we found that appropriate checks were not always undertaken before staff began work. We found that criminal record checks and references had not been obtained for all members of staff.

We also found that clinical notes were stored electronically but that clinical notes lacked sufficient detail and did not provide a clear account of each patient's appointment, what treatment options were discussed or what treatment was carried out.

During this follow up inspection on the 11 December 2013 we spoke with the practice manager and the dentist at the practice. The practice consists of one dentist, the practice manager and two dental nurses.

At our inspection on the 5 August 2013, we noted that there were no details available for patients about the fees charged for treatments. During this follow up inspection, we noted that the provider had a list of fees in the reception area available for patients to look at.

During this inspection, we focused on whether the provider had met the compliance actions in respect of Outcome 12 requirements relating to workers, Regulation 21 (HSCA 2008 Regulated Activities) Regulations 2010 and Outcome 21 Records, Regulation 20 (HSCA 2008 Regulated Activities) Regulations 2010 following our compliance actions.

During a routine inspection

During our inspection we spoke with six patients and four members of staff which included the principal dentist and practice manager.

We found that patients understood the care and treatment choices available to them. All the patients we spoke with told us that they were satisfied with the care and treatment received. One patient told us that they were 'very happy and would go back again for treatment'. Two patients told us that they had such a good experience that they had recommended the practice to their friends and family.

The premises was clean and welcoming. There were arrangements for the prevention of infection and staff we spoke with told us that they had received training in infection control and the decontamination procedures for equipment and instruments used.

We saw that the practice had a safeguarding policy and whistleblowing policy. The provider informed us that two members of staff had not yet received safeguarding training and that these staff would attend safeguarding training within the next two months.

We looked at three staff files and noted that criminal record checks had not been carried out on two members of staff.

We also looked at four patient files and saw that there were insufficient clinical notes. These notes failed to provide details of each appointment the patient had and what options had been discussed as well as the treatment that had been carried out.
