Well-led
We found this practice was not providing well led care in accordance with the relevant regulations. The provider had made insufficient improvements to put right the shortfalls and had not responded to all of the regulatory breaches we found at our assessment on 24 July 2024. During our assessment of this key question, we found: A lack of systems or processes and some ineffective systems that enabled the registered person to assess, monitor and improve the quality and safety of the services being provided. A lack of systems or processes to assess, monitor and mitigate the risks relating to the health, safety and welfare of service users and others who may be at risk. This resulted in a breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. We have told the provider to take action. We will be following up on our concerns to ensure they have been put right by the provider. The impact of our concerns, in terms of the safety of clinical care, is minor for patients using the service. Once the shortcomings have been put right the likelihood of them occurring in the future is low.

Governance, management and sustainability
We found there was a continued lack of leadership and oversight to assess, monitor and improve the quality and safety of services provided, which led to the issues highlighted by the on-site assessment. However, we found staff to be open to discussion and feedback to make the necessary improvements to ensure people’s safety. We have seen evidence of some improvements since our last visit. We were not assured that staff responsible for the ongoing monitoring of water systems were competent nor did they have the appropriate training or knowledge to do so effectively. Improvements had been made to clarify staff responsibilities, roles and systems of accountability.
Following on from our last visit, systems and processes were still not embedded, and the assessment still highlighted issues and omissions. There was limited improvement from the previous assessment. The governance system including policies, protocols and procedures was not effective nor reviewed on a regular basis. Systems and processes for ensuring staff training was up-to-date and completed at required intervals still required improvement. There was no improvement to oversight and assurance processes for ensuring all staff recruited had appropriate documentation, such as Disclosure and Barring Service checks and effective vaccination history and effectiveness. There was a lack of improvement in systems to assess, monitor and improve the quality and safety of the services being provided. Required infection prevention and control audits and record-keeping audits that were highlighted as missing at the previous assessment had still not been completed. Antimicrobial prescribing audits had also not been completed and the existing radiography audit which was ineffective had not been redone. However, improvements had been made to systems to enable timely response to a medical emergency. Items previously missing or expired in the medical emergency kits had been replaced and a checklist implemented to ensure this would not happen again. Improvements had been made to the oversight and knowledge of the requirements of the Resuscitation Council UK guidance.
