Governance and oversight systems were not always effective and internal audits had not identified the concerns found in relation to infection control, maintenance of the building and medicines. The service was mainly operating as a respite service, people attended the providers day services in another location during the day. There was a positive culture at the service and staff and relatives we spoke with were happy with how the service was run.

Shared direction and culture

Score: 3

Staff were positive about the culture of the service. A staff member told us when we asked about the vision and values at the service, “We promote independence, offer choice, support people with basic life skills and social skills, it is well managed and managers are visible.” Another staff member said, “I am in awe of this place because of what [registered manager] has done.”

Whilst the relatives and staff we spoke with were positive about the culture at the service and processes in place to support people and staff were good. Staff spoke about people’s rights passionately and worked well to ensure people had a positive experience whilst staying at the service. However, staff and leaders had not recognised the environment people were living in was not clean or well maintained.

Capable, compassionate and inclusive leaders

Staff were positive about the support they received from senior staff. A staff member told us, “I do have supervision and I do feel supported. We all get on there are no problems.”

Whilst there were processes in place to support staff such as supervision, appraisal and staff meetings. Senior leaders had not picked up the concerns we found and more learning was needed to ensure the manager was competent with auditing processes.

Freedom to speak up

Staff told us they were confident to speak up and managers listened to what they had to say.

The manager and registered manager were visible in the service and it was clear they knew staff and people well.

Governance, management and sustainability

Score: 1

Staff told us they thought the service was well managed. However, whilst the registered manager told us they discussed cleanliness with staff in meetings we found no action had been taken to ensure the service was clean and well maintained.

The providers' quality assurance systems in place were not effective in identifying the concerns found with infection prevention and control, medicines and maintenance of the building. Audits in relation to infection control, maintenance and catering had been undertaken in July 2024 and not identified the concerns found. The registered manager told us during the feedback session they had already started working on cleanliness and the environment.

Learning, improvement and innovation

Score: 2

Whilst we did see areas where previous concerns had been actioned by the registered manager and staff supported with additional training and support. We found improvements were needed to ensure all staff were fully aware of their responsibilities in relation to providing quality care for people in a clean and supportive environment. Staff told us they supported people to be involved in all aspects of the service including the cleaning, however the registered manager had not evaluated if this approach was working effectively.

We were not assured senior staff were always recognising when improvements were needed particularly in relation to the environment. However, it was positive to note staff told us they were able to speak up and their ideas were listened to.
