Well-led
Roles and responsibilities for management and leadership were clear and known by all staff. The service monitored performance and where opportunities to improve existed, changes were made. Data was used to help make decisions and monitor progress. Governance systems and processes ensured the service acted on the best information and benefitted patients.

Governance, management and sustainability
We spoke with staff in a range of roles including leadership, management, and support and received completed staff feedback forms. All were clear about their roles and responsibilities. We also reviewed minutes of meetings for a complete range of staff groups. These demonstrated that staff communicated effectively within the practice and there were established processes to discuss risk, performance, and concerns. Meeting minutes and other documents demonstrated that the practice had a strong team to help ensure the service delivered high quality care and would be sustainable for the future.
Governance systems and processes existed within the practice. For example, performance and risk was monitored and there were systems to receive feedback and complaints from patients. We saw that audits, staff, patient and Patient Participation Group (PPG) feedback had been completed and considered. Leadership provided evidence to show new appointments and new systems had been introduced following patient feedback regarding access to appointments with nurses, access to appointments on a Saturday and urgent access to appointments / advice for unwell children under the age of 2 years. The practice had clear and comprehensive processes to receive feedback from patients, stakeholders and staff.
