We carried out this inspection under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. This inspection was planned to check whether the service was meeting the legal requirements and regulations associated with the Health and Social Care Act 2008. We looked at 5 of the priority quality statements that included for this assessment, safe and effective staffing, medicines optimisation, safe systems and governance and assurance. This service is registered with CQC under the Health and Social Care Act 2008 in respect of some, but not all, of the services it provides. There are some exemptions from regulation by CQC which relate to particular types of service and these are set out in Schedule 2 of The Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. At Castle House those occupational health related services provided to customers under arrangements made by their employer or a government department are exempt by law from CQC regulation. Therefore they did not fall into the scope of our inspection. We found no breaches of regulations.

Delivering evidence-based care and treatment

The provider told us that they assessed needs and delivered care in line with relevant and current evidence based guidance and standards such as Public Health England and the National Travel Health Network and Centre (NaTHNaC). NaTHNaC is a body set up to protect the health of British travelers and improve the quality of travel health advice given by GP practices, travel clinics, pharmacies and other healthcare providers. It provides up-to-date and reliable information for the travelers, travel industry and national government. Staff told us that people had face to face consultations with healthcare professionals to discuss their travel plans and health needs. Staff ensured a full medical history was taken before making recommendations for what treatments were needed. People were given written information about vaccines and other medicines and were signposted to external support where needed. Staff informed people if medicines were available on the NHS. Consent was sought before any medicine was administered. As part of the initial health check prior to vaccinations offered, it was determined if the client had recently undergone medical treatment or had a disorder or disease that caused any immunosuppression. If this was determined to be applicable, then the service’s clinical staff would seek permission to contact the client’s GP or consultant.

The provider told us that they completed risk assessments. We saw that they were clear, comprehensive and up to date. They contained enough information about people’s risks and mitigation strategies for staff to provide safe and effective care. Environmental risks were managed safely, including fire, water temperatures, infection control and maintenance of equipment. Clients could access the service privately, via their employer or occupation health scheme. There was no direct information sharing of vaccination status with the patient’s GP. However, people were given a vaccination card which included all the relevant information, and the patient was advised to share this with their GP. Where treatment was given by other healthcare providers there was a process to follow up and gather appropriate information to continue treatment.
