About the service

Yorkshire Homecare Limited is a domiciliary care service providing personal care to adults with a range of support needs in their own homes. At the time of our inspection there were 6 people using the service.

Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do we also consider any wider social care provided.

People’s experience of using this service and what we found

Systems and processes were in place to safeguard people from the risk of abuse. Relatives told us people received safe care and there were enough staff to meet people’s needs. Medicines were generally managed well. Staff had a good understanding of infection prevention control. The provider ensured staff were recruited safely and received supervision and support to ensure safe care.

The registered manager ensured regular reviews of peoples’ needs. Relatives described ways they were involved in the development and updating of care plans and risk assessments. Staff had a robust induction at the start of their employment, further coaching and competency checks ensured they had the skills and knowledge to care for people safely and in a person-centred way. Staff understood how to provide personalised care and ensure people’s nutritional needs wet met.

People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

The registered manager responded to feedback, ensuring lessons learnt were being used to improve the service. There were systems in place to monitor that people received safe and good quality care. Relatives and staff had good experiences of the service and could contribute to further improvement within the service.

Rating at last inspection and update

The last rating for this service was requires improvement (published 13 October 2021). The provider completed an action plan after the last inspection to show what they would do and by when to improve. At this inspection we found improvements had been made and the provider was no longer in breach of regulations.

Why we inspected

We carried out an unannounced comprehensive inspection of this service on 13 October 2021. Breaches of legal requirements were found. The provider completed an action plan after the last inspection to show what they would do and by when to improve safe care and treatment, good governance and staffing.

We undertook this focused inspection to check they had followed their action plan and to confirm they now met legal requirements. This report only covers our findings in relation to the Key Questions Safe, Effective and Well-led which contain those requirements.

For those key questions not inspected, we used the ratings awarded at the last inspection to calculate the overall rating. The overall rating for the service has changed from requires improvement to good. This is based on the findings at this inspection.

During a routine inspection

Yorkshire Homecare Limited is a domiciliary care service which provides personal care to adults with a range of support needs in their own homes. At the time of this inspection the service was supporting 20 people.

Not everyone who used the service received personal care. CQC only inspects where people receive personal care. This is help with tasks related to personal hygiene and eating. Where they do we also consider any wider social care provided.

This inspection took place between 28 June 2021 and 31 August 2021. This inspection was announced.

People’s experience of using this service and what we found

Governance systems were not always in place to assess, monitor and improve the quality and safety of the service provided. We also found shortfalls in a number of areas relating to record keeping and audits in the service.

Medicines were not always managed safely because staff had not received competency assessments and not everyone had completed their medicines training. Staff did not always record when medicines had been administered. Guidance for staff in the form of protocols was not always in place to instruct staff on how to administer medicines.

Staffing levels were sufficient however, people and their relatives told us their visits were often late or not at the allocated times. Most recruitment checks were completed to ensure people were safe to work with people using the service. However, one staff member had started working prior to their DBS being obtained. Staff training and supervisions had not always been completed in line with the providers policy.

Safeguarding and incidents had been investigated and were being managed effectively. However, we found one incident where the actions taken had not been updated in the person’s risk assessment. All other risk assessments were completed, reviewed and changed with people’s care needs.

People using the service told us they felt safe and staff followed the provider’s safeguarding policy for reporting and acting on concerns. Staff felt confident to raise concerns if needed.

Staff understood the requirements of the Mental Capacity Act 2005. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible.

People using the service said staff were caring, helpful and respected their wishes. Staff ensured they always offered choice and encouraged people to remain independent when their health allowed.

Complaints were managed with actions taken to address the concerns and people felt their concerns would be responded to. There had been minimal complaints received by the service since they opened.

People living in the home, their relatives and staff spoke positively about the current management of the service.

Why we inspected

The service was registered with us on 2 January 2019 and this is the first inspection.

During a routine inspection

Yorkshire Homecare Limited is a domiciliary care agency providing care to people in their own homes and flats. This service was registered with the Care Quality Commission in January 2019, although had only provided very limited personal care since registration. At the time of the inspection three people were receiving this service. We have been unable to allocate a rating to our key questions to give an overall rating as there was insufficient service history for us to form a judgement.

People's experience of this service and what we found

Recruitment systems were in place to ensure staff had suitability checks before working with people. Risks were identified for individuals, although needed more detail to give staff clear guidance. Safe procedures were in place to support people with medicines, although recording needed to be clearer. The provider showed us how new documentation was being introduced to include more detail in records. Accidents, incidents and safeguarding documentation and procedures were in place, although nothing had happened to record.

Feedback from people showed confidence in staff skills to do their work. Systems were in place for staff training, appraisal and supervision.

Mental capacity information was in place to support people’s rights. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice.

People said staff had a caring attitude and knew their individual needs well.Staff were reliable, kind and respectful in people’s homes. Care documentation was being implemented and improved for staff to have clearer information about each person.

There was a process in place to address concerns and complaints; the provider confirmed none had been received about the service so far. Policies and procedures related to the regulations although contact numbers needed to be included, such as for safeguarding.

The service was in the early stages of development and therefore had not had time to demonstrate how effective the systems and processes were. No formal staff meetings had been established due to the size of the service. The provider and manager had a shared clear vision about the direction of the service and understood their responsibilities.

The service was registered with us on 9 January 2019 and this is the first inspection.
