During an inspection looking at part of the service
We found assessments of people's needs were not always thorough and confirmation as to why treatment was necessary was not always evident.
People who use the service were not always protected against the risks of unsafe or unsuitable premises because appropriate risk assessments had not been undertaken.
The provider did not have a safeguarding policy in place and staff had not undertaken appropriate safeguarding training.
People were not protected from the risk of infection because appropriate guidance had not been followed.
Staff were not always supported to deliver care and treatment safely and to an appropriate standard because training was not provided.
The provider did not have an effective system to regularly assess and monitor the quality of service that people receive.
