Staff understood their role and responsibilities. Staff had received the appropriate training and were receiving regular supervision and appraisal. There were systems in place to check performance and compliance with the assessment, planning and evaluation of clients’ care and treatment. The provider had effective governance framework in place, which was implemented through a series of structured meetings. The governance processes in place operated effectively at team level, and performance and risk were managed well. The service was proactive in identifying learning needs of the staff to refresh and enhance practice. Managers completed a range of audits to ensure that the service was safe and effective such as clinical audits, client involvement and client file audits.

Governance, management and sustainability

Staff understood their role and responsibilities. Staff had received the appropriate training and received regular supervision and appraisal. Staffs’ compliance with mandatory training was recorded and the manager could view the records on a dashboard. Overall, 96% of permanent staff had completed mandatory training. There were systems in place to check performance and compliance with the assessment, planning and evaluation of clients’ care and treatment. Managers completed a range of audits to ensure that the service was safe and effective such as clinical audits, client involvement and client file audits. The provider held a monthly performance meeting which explored a range of areas such as self-discharge, medicine’s recording errors and failed admissions.

The provider had effective governance processes in place, which was implemented through a series of structured meetings. The governance processes in place operated effectively at team level, and performance and risk were managed well. The provider had systems in place to manage performance, risks and quality of the service provision. Information was used effectively to monitor and improve the quality of care. Governance policies, procedures and protocols were regularly reviewed and were up to date. At the previous inspection in July 2021, the provider did not consistently notify CQC of notifiable incidents. At this assessment, we found that the provider was compliant and notifications were consistently submitted to CQC as required. The service had a risk register in place which fed into a corporate risk register for the provider. Managers attended regular meetings to support governance and the day to day running of the service.

Learning, improvement and innovation

Staff told us that they received regular supervision and supported to develop and improve service. Staff told us that they felt confident about raising concerns regarding the service. Staff felt their voice was listened to and their suggestions were used to improve the service. Staff at all levels had opportunities to learn, and poor performance was managed appropriately.

Managers completed a range of audits to ensure that the service was safe and effective such as clinical audits, client involvement and client file audits. The service had a medication error action plan in place due to number of recent medicines errors. The service was proactive in identifying learning needs of the staff to refresh and enhance practice. A registered nurse from another provider site recently delivered medicines training and medicines competency training to staff for over four days. This covered medicines administration, medicines audit and medicine room management. Completion of clinical audits had led to positive improvements in other areas such as risk assessments, care planning, and observation competency.
