The service had effective governance processes in place. Staff recognised incidents and reported them appropriately. Managers investigated incidents and shared lessons learned with the whole team and wider service. Managers ensured that staff received training and appraisal.

Capable, compassionate and inclusive leaders

Staff told us that leaders were visible and approachable. Staff told us that they felt valued and that they had seen significant improvements in the last 6 months.

Leaders demonstrated that they were aware of examples of poor culture that affected the quality of people’s care and had a detrimental impact on staff and were making efforts to address these. Leaders demonstrated that they were knowledgeable about issues and priorities for the quality of services and could access appropriate support and development in their role.

Governance, management and sustainability

Score: 3

A structured governance framework was in place. Managers had oversight of the risks within the service and had put measures in place to make improvements. Senior leaders had supported the manager to put a comprehensive action plan in place to address concerns regarding missed observations. However, staff had not received regular supervision. Managers were aware and had put measures in place to address this.

Staff demonstrated that they understood their role and responsibilities. Managers accounted for the actions, behaviours and performance of staff. Staff told us that they received supervision but not as frequently as they would have liked. A ward manager had recently been appointed to provide an increased leadership presence.
