During an inspection looking at part of the service

We undertook a follow up focused inspection of Cricklewood Dental Clinic on 2 November 2022. This inspection was carried out to review in detail the actions taken by the registered provider to improve the quality of care and to confirm that the practice was now meeting legal requirements.

The inspection was led by a CQC inspector who had was supported by a specialist dental advisor.

We undertook a focused inspection of Cricklewood Dental Clinic on 23 July 2021 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We found the registered provider was not providing safe and well led care and was in breach of regulations 12 and 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. At a follow up inspection on 9 December 2021 we found that the provider had made some improvements but had not taken action to fully comply with the regulations. When one or more of the 5 questions are not met we require the service to make improvements and send us an action plan. We then inspect again after a reasonable interval, focusing on the areas where improvement was required.

We found this practice was providing safe care in accordance with the relevant regulations.

Background

The provider has 3 practices and this report is about Cricklewood Dental Clinic.

Cricklewood Dental Clinic is in the London Borough of Brent and provides NHS and private dental care and treatment for adults and children.

The dental team includes the principal dentist, 1 associate dentist, 1 dental therapist and 1 trainee dental nurse. The clinical team are supported by a receptionist and a practice manager. The practice has 1 treatment room and a separate decontamination room.

During the inspection we spoke with the principal dentist, the receptionist and the practice manager. We looked at practice policies and procedures and other records about how the service is managed.

The practice is open:

Monday to Friday 9am to 6pm.

We carried out this announced inspection on 9 December 2021 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to follow up on concerns we identified during our inspection of the service on 23 July 2021 and to check whether the registered provider was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations.

The inspection was led by a Care Quality Commission (CQC) inspector who was supported by a specialist dental adviser.

We undertook a comprehensive inspection of Cricklewood Dental Clinic on 23 July 2021 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We found the registered provider was not providing safe or well led care and was in breach of regulations 12 – Safe care and treatment and 17 – Good governance.

The provider had made some improvements. However, they had failed to make all of the improvements in relation to the regulatory breaches we found at our inspection on 23 July 2021.

The practice is owned by an individual who is the principal dentist there. They have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated regulations about how the practice is run.

During the inspection we spoke with the principal dentist, the associate dentist, the trainee dental nurse and the receptionist. We looked at practice policies and procedures and other records about how the service is managed.

The practice opening times are:

9am to 6pm Mondays to Fridays

Our key findings were:

  • The provider had arrangements to ensure that equipment was tested, serviced and maintained in accordance with relevant guidelines.
  • Infection prevention and control procedures were in accordance with national guidance. However, there were ineffective arrangements to monitor these procedures.
  • Emergency equipment was not available in accordance with the Resuscitation Council UK 2021 guidelines.
  • There were ineffective systems to assess and mitigate the risks of fire at the practice. The findings from the most recent fire safety risk assessment had not been acted on to reduce identified risks.
  • The provider had ineffective procedures to ensure that all the required information was available for staff who work at the practice.
  • There was ineffective leadership to support a culture of continuous improvement, and there were ineffective governance systems to monitor the day to day running of the practice.

  • The provider had safeguarding processes and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • The provider had staff recruitment procedures which reflected current legislation. Improvements were needed to ensure that all the required employment checks were carried out and records of these were maintained.
  • The clinical staff provided patients’ care and treatment in line with current guidelines.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • The practice appeared to be visibly clean and well-maintained.
  • The provider had arrangements to manage risks of COVID-19 virus in accordance with current guidelines.
  • Infection prevention and control procedures were not followed in accordance with national guidance.
  • Staff knew how to deal with emergencies. Emergency equipment and medicines were not available in accordance with the Resuscitation Council UK 2021 guidelines.
  • The provider had ineffective systems to help them manage risks to patients and staff.
  • The provider had ineffective leadership to support a culture of openness and continuous improvement.
  • There were ineffective governance systems to monitor the day to day running of the practice.

We identified regulations the provider was not complying with. They must:

  • Ensure care and treatment is provided in a safe way to patients.
  • Establish effective systems and processes to ensure good governance in accordance with the fundamental standards of care.

During a routine inspection

Patients who used the dental practice were positive about the quality of dental service they had received. For example, patients' told us she is a good dentist, she always explains things'. They told us that treatment plans were explained to them in a way they understood and the costs of treatment were made clear before treatment commenced.

Patients said that their privacy and dignity were respected by the dentist.

Patients told us that they felt safe and that the dental practice was always clean and tidy.
