During a routine inspection

We carried out this inspection to review the improvements made following our inspection in July 2013 and to check on the care and welfare of people using this service. We found shortfalls in all of the outcomes we assessed in July 2013 and told the provider to take action to put things right. The provider sent us a report setting out the action they would take to meet the standards.

People shared positive experiences of their care and treatment. Everyone was satisfied with the treatment they had received and told us they would and had recommended the practice to their family and friends.

We saw arrangements for the safe management of infection control had improved to reduce the risk of cross infection.

Two new permanent staff had been employed and there were opportunities for staff to meet regularly as a team. Not all staff had received refresher training in safe working practices. The provider acted on this shortfall following our inspection.

Systems to monitor the quality of the service had been implemented and opportunity to gain feedback from people had been introduced.

Not all of the patients we spoke with were familiar with the formal complaints procedure. However, they told us they would feel confident in raising concerns directly with the practice staff.

Records we sampled were detailed and more accurately maintained.

During a routine inspection

We observed people being greeted on arrival at the practice. We saw people were dealt with respectfully and were offered appointments on days and times to suit them. People we spoke with were generally satisfied with their treatment. People commented that they felt options could be explained in more detail so that they understood better.

The provider and staff had received training to deal with medical emergencies. However; they were not familiar with the usage of the new emergency equipment obtained. This meant staff may not be competent in the event of a medical emergency which may put people at risk of harm.

People received treatment in a clean environment. Records for the frequency of cleaning schedules and setting up and closing down of the surgeries each day were not regularly signed by staff. These records had gaps which meant that certain tasks may have been omitted. There was no evidence of provider audit of decontamination processes.

The provider had obtained professional external support in the management of the practice. We saw a number of quality audits had recently been carried out but the provider was unable to demonstrate the service was managed effectively and quality was maintained.

People could not be assured that complaints had been listened to seriously or learnt from.

People were not protected from the risks of unsafe or inappropriate care and treatment because accurate and appropriate records were not maintained.
