During an inspection looking at part of the service

This report is a follow up to our report published in April 2013. We had inspected this provider on 6 March 2013, when we noted some issues which had a minor impact on people using the service. These related to monitoring the quality of the service. The provider did not ensure that there were effective quality checking systems. We set compliance actions requiring the provider to take steps to comply with Regulations 10 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010.

We made a further visit on 24 July 2013 to check on the action the provider had taken to meet the requirements of the regulations. We found that the provider had a working system in place to regularly assess and monitor the quality of service that people received.

During an inspection looking at part of the service

This inspection was undertaken to look at the progress Ivy House Dental Practice had made since we visited on 25 October 2012 and found that it was not meeting four standards. This time we found it was compliant with three standards, but was still not meeting one standard.

We used a number of different methods to help us understand the experiences of people using this service. When we visited last time we spoke with two patients. Neither raised any concerns with us regarding their care. This time we looked at patient feedback forms and the surgery comments book.

The provider had now taken reasonable steps to identify the possibility of abuse and prevent abuse from happening.

The provider had undertaken checks on staff and had a system in place to ensure that all staff recruited in the future had appropriate checks undertaken on them prior to starting work.

The provider had taken steps to ensure that the training needs of staff were identified and that they were supported to receive appropriate professional development.

We found that the provider did not yet have a working system in place to regularly assess and monitor the quality of service that people received.

During an inspection in response to concerns

We spoke with two patients at the practice. They spoke positively about the quality of service they had received. They told us staff treated them with respect and dignity. They said their treatment plans were explained to them in a way they understood and the costs of treatment were made clear to them before their treatment started. They told us their privacy was respected. Their comments included the following:

"I come to this surgery, as well as my family, and we are all happy with the treatment we receive'

"The staff are very polite and helpful. They have explained the treatment I need"

We found evidence that patients received appropriate information about their treatment and its costs. This enabled them to make an informed decision about their care. We also found that the practice had procedures in place to ensure people were protected against the risks of infection.

We found that the practice did not have the appropriate policies and procedures in place to ensure staff would be able to identify the possibility of abuse and prevent it from happening. The practice did not have evidence it had undertaken appropriate recruitment checks on staff prior to their starting work. There was also insufficient evidence that staff were receiving all the training they needed for them to undertake their role. The practice did not have sufficiently robust systems to ensure it was identifying problems and addressing them.
