Our view of the service

Date of assessment 25 March to 15 April 2024. Harmony House is a nursing home providing accommodation for up to 57 older people who require nursing or personal care. This was the first assessment of Harmony House under the new provider, Rosefield Care Limited which was registered with the Care Quality Commission on 9 August 2023. This assessment was brought forward due to information of concern about the management of risks related to people's health, manual handling and good governance. As a result, this assessment only examined this risk and assessed 14 quality statements which included safeguarding, involving people to manage risk, the providers learning culture, safe environment, infection prevention and control, assessing needs, delivering evidence-based care and treatment, how staff teams and services work together, equity in experiences and outcomes, capable, compassionate and inclusive leaders, freedom to speak up, governance and assurance, partnerships and communities and learning, improvement and innovation. We will assess the other quality statements in future assessments, at which point, the home will receive a rating. We found 2 breaches of the regulations in relation to safe care and treatment, and good governance. Risks related to people’s care were not always managed well. Risk assessments did not always contain sufficient detail to ensure staff had the information required to understand and mitigate specific risks to people such as eating and drinking. Risks in the environment were not always managed safely. Some fire doors were ineffective and cleaning chemicals had been left unattended in an unlocked room. The providers systems and processes had failed to identify, manage and mitigate some of these risks.

People's experience of this service

We spent time observing staff interactions with people and spoke with 5 people and 14 relatives about their experience of the care provided. Overall, people and their relatives were positive about the general care provided at Harmony House and made positive comments about the staff who provided their care. People and relatives felt comfortable to raise any concerns they had with the registered manager and were confident their concerns would be addressed. People and relatives told us the home was always clean and tidy. People and relatives told us people were safeguarded from the risk of abuse. Comments included, "I have absolutely no concerns about their safety here. I see everything that goes on", "I’ve no concerns at all, the girls are all lovely" and, "I’m happy here the girls will do anything for you. I feel quite safe, it’s knowing there’s always someone there." However, some people and relatives felt more could be done to promote people's emotional well-being. One person told us, "I’d like to go out more, have lunch in the dining room but they never take me. The only time I see staff is when they give me my medication and change my pads, otherwise I just lie here all day. I've asked staff to get me up but they say no. They are not bothered about me." A relative commented, "One nurse gets them out of bed the others don’t bother. One day I asked why they were still in bed and she said they hadn’t got time." People and relatives felt people's general health needs were met but did not always feel involved with the plan of care. Comments included, "They look after any medical conditions alright but they never discuss my care needs," "They’ve never discussed [person's] care with me. I don’t know about care plans" and, "They don’t discuss [person's] care with me. I've never been involved with a care plan. I don’t think they have time to listen’.
